The browser microphone records; the live transcript labels physician and patient separately.
When the consultation ends, complaint, history, examination findings, assessment and plan are drafted. Greetings and small talk stay out of the note.
The physician edits and signs the draft; the note is locked and the audio is deleted.

Anamnesis draft
Targets: a draft within 60 seconds for a 15-minute consultation; drug names, doses and measurements carried over verbatim; every sentence has a source in the transcript.
Audio is permanently deleted within 5 minutes of note sign-off, or within 24 hours at the latest if the note is not signed. Deletion is audited.
If the patient objects, "Cancel recording" deletes the audio and transcript within 1 minute.
Every sentence in the note links to its place in the transcript; a sentence without a source is highlighted.
The form of consent and the retention period are the institution’s decision; Memoris is the processor.
